Bio

Teresa Eckes, LICSW, supports the clinic and patients in a variety of ways. These include connecting patients to specialties, assisting with obtaining durable medical equipment, arranging home care and hospice, assists with Health Care Directives, and connection with diagnosis specific agencies (ex. Alzheimer’s Association). She also helps patients with community resources, such as insurance, housing, financial, mental health and safety planning.
