    University of Minnesota Physicians Outreach Laboratories specialize in innovative, complex, diagnostic laboratory testing. Through clinical and interpretative medical direction from pathologists, doctoral scientists and other physicians, we ensure the highest quality of results.

Patients with urgent medical or surgical needs are given priority through our appointment system. Appointments for non-urgent problems are scheduled in advance at the first possible date. We accept emergency cases to the fullest extent of our resources. Patients requiring hospitalization may be referred to an emergency room for evaluation.

Please be prepared to provide the following information when scheduling an appointment:

  • Patient's name, gender, address, phone number, date of birth
  • Reason for the referral, symptoms and diagnosis (including services you would like University of Minnesota Physicians to provide)
  • Referring Physician information (UPIN/NPI, physician name, clinic, phone and fax number, address, contact name)

HMO and PPO patients are expected to obtain a referral for a visit if required by their insurance plan and must pay the copayment specified by the plan at the time of service. Please have your patient familiarize themselves with their specific coverage to understand their payment obligation. They should contact their insurance company to determine their plan's specific requirements and coverage options.

For bordering states: As participants of Minnesota border-state Medicaid programs, we manage care plans through state Medicaid programs. Referrals or prior authorizations are required in order for services to be covered. For questions regarding insurance coverage and benefits, referrals, prior authorizations, and any potential out-of-pocket costs, please contact your insurance company directly. Patients are required to bring their current Medical Assistance card to their appointment. If the plan requires a copayment, it will be due at the time of service.

For non-bordering states: We will not accept Medicaid insurance as coverage for services. Exceptions to this include emergency services and arrangements that are made in advance to fully pay billed charges for both the physicians and hospital services.
